Family Case Management

Family Case Management (FCM) is a program which provides supportive services to low-income pregnant women and infants (children under one year of age). Expecting a baby is a life-changing experience. Good prenatal care may prevent premature birth of the baby and possible medical complications. We provide you with information and referrals of resources that are available so that you can be self-sufficient for the benefit of your family. Family Case Management supportive services are available to low-income pregnant women and infants under one year of age.

FCM Services include but are not limited to the following:

  • Medical, social service, or developmental needs assessments
  • Referrals available for the following services:

    • Preventive health education
    • Prenatal classes
    • Direction toward food pantries
    • Mental health
    • Domestic violence prevention
    • Substance abuse treatment and prevention
    • Emergency housing
    • Transportation services

Follow-up during the first year of the baby’s life

Family Case Managers offer guidance and support to families so that they may have healthy babies and lead healthy lives. Quality medical care, good nutrition and emotional health in the first year of a baby’s life may help them reach healthy development. It is very important to have a pediatrician to provide medical follow-ups and timely immunizations. Case Managers will help you understand proper nutrition for you and your child as well as understand the stages of your child’s development. Specialized staff assess psychomotor development and the baby’s weight on a quarterly basis.

Follow-up services include:

  • Guidance by Case Managers on proper nutrition for you and your child
  • Education resources to understand your child’s development
  • Specialized staff assess psychomotor development and the baby’s weight on a quarterly basis
  • Guidance on treatment options for post-partum depression

All services are bilingual and bi-cultural.

Family case management is an Illinois Department of Human Services program to promote healthy families and reduce maternal and infant mortality. Funding provided by the Illinois Department of Human Services.
