Medical Records

All Alivio patients can request a copy of their medical records; however, proper release of your information requires appropriate authorization. Release for medical records may only be given by a patient.

Download a copy of the medical records request form to print, complete and return to us via mail or in person at one of our clinics to obtain a copy of your medical records.
