Medical Records

All Alivio patients may request a copy of their medical records; however, proper release of your information requires appropriate authorization. Release for medical records may only be given by a patient.

Download a copy of the medical records request form to print, complete and return to us via mail, fax, or in person at one of our clinics to obtain a copy of your medical records. When picking up records, you will be asked to provide a state or federally issued photo ID. Unless prior arrangements have been made, we will not release information to individuals other than the patient or insurance guarantor.
